Lookup banyak Kriteria

Bagaimana membuat lookup di excel dengan banyak kriteria, salah satu cara yaitu dengan menggunakan fungsi INDEX dan MATCH, Berikut contoh soalnya :
Dari contoh soal di atas diminta jawaban dengan formula yang di letakkan di cell (i3), berikut formulanya
{=INDEX($D$3:$D$5,MATCH(F3&G3&H3,$A$3:$A$5&$B$3:$B$5&$C$3:$C$5,0))
Keterangan :
INDEX($D$3:$D$5 Hasil yang ingin ditampilkan
MATCH fungsi yang akan mencocokkan
F3&G3&H3 Kriteria dari soal
$A$3:$A$5&$B$3:$B$5&$C$3:$C$5 kriteria dari tabel kriteria
0 indikasi untuk kesalahan
